Slippery Rock University Athletics

All Student-Athletes MUST complete all medical requirements prior to being permitted to participate in athletics at Slippery Rock University.  Late forms could result in delayed clearance for athletic participation. 

Checklist for NEW students:

• Sickle Cell Trait Blood Results
  • For student-athletes who were born in the state of PA (click here)
  • For student-athletes who were not born in the state of PA (click here)
     
• Physical to be completed on the SRU Pre-Participation Physical Examination Form: (Click Here)
  • Please note Physicals have to be done within 6 months of first day of practice.
